RISING STARS BASKETBALL TERMS AND CONDITIONS
In consideration of the acceptance of my application
for the Rising Stars Camp (Hereinafter, called RS),
I hereby waive, release, and discharge any and all
claims for damages for personal injury, property
damages or which may hereafter occur to me as a result
of participation in said RS event.
Medical Conditions:
All member should consult with his/her physician
before allowing their children to use the services
provided by RS or participating in any seasons,
sessions or events hosted or conducted by RS.
Member hereby represents and warrants to RS staff
that Member's child (and Member) is in good health
and has no disability, impairment, injury, disease,
or ailment that prevents Member from engaging in
active high-intensity exercise, basketball games,
and instruction.
Member understands and acknowledges that neither RS,
nor any trainer, coach, member, director, officer,
employee, school, contractor, advisor or representative
of RS has any expertise in diagnosing, examining or
treating any medical condition.
It is understood that some recreational activities,
camps, and sports involve an element of risk or danger
of accidents, even accidental death, and knowing those
risks, I hereby assume those risks.
I understand that pictures and videos taken during
training sessions, games and camps can be used for
future promotional use.
Refunds and fees:
If a Rising Stars basketball session needs to be
cancelled because of unforeseen circumstances
(ie: weather, COVID-19) we will issue a credit for
any session in the future.
Consent for Treatment:
I hereby give my consent to have the above applicant
treated by emergency medical personnel, a physician,
or surgeon, in the case of sudden illness or injury
while participating in the above activity.
